Advance Care Planning Conversation

In the event of a medical emergency, what measures do you want medical professionals to take on your behalf, and who do you want to make decisions for you if you are unable to direct your care yourself? Have you documented your wishes, to protect yourself and those you love, in the event of a future medical emergency? Advanced care planning is something that must be done before an emergency arises, and because it can be a difficult topic it is often left undone. UnityPoint at Home would like to help. We will be hosting an event to facilitate conversations regarding Advance Care Planning. The event is open to the public, free of charge, and a light meal will be provided. Bring a spouse, parent, friend, or other loved one! Prepare to laugh, contemplate, and maybe even cry. Event will be held Tuesday, April 25th from 5:30-7:30 at St. Paul's Lutheran Church.
